My fees are $190.00 for the initial assessment and $165.00 for regular sessions. If you are using insurance for your sessions, your fee will be based on the negotiated rate for that type of insurance.

I accept the following insurance plans:

• Wellmark Blue Cross/Blue Shield (but not Common Spirit, Mercy CHI Employees, or Value Plans)
• Federal Blue Cross/Blue Shield
• Most Out of State Blue Cross/Blue Shield plans
• Health Partners Unity Point
• Health Partners Minnesota

Please contact your insurance carrier for the following information:

1. If your insurance plan covers me.
2. If your insurance plan has a deductible that includes mental health appointments. A deductible is the amount you will pay out of pocket before your insurance pays all or part of your appointment costs.
3. If you have a co-payment amount or co-insurance percentage. This is the amount you will owe each session.
4. If your plan will cover telehealth sessions provided by me. Some insurance plans require you to see a contracted telehealth provider such as Doctor on Demand or Teledoc.

Please note that you are responsible for co-payments, co-insurance, and/or deductibles as indicated by your individual policy. Payment is expected in full at the end of each session.
